Eastern Health’s Supportive and Palliative Care Service is seeking an enthusiastic and suitably qualified/experienced Nurse Practitioner or advanced Nurse Practitioner Candidate (i.e. maximum of 6-12 months pre-endorsement) to take a leading role in the development and trial of a new Palliative Residential Aged Care Consult (PRACC) Service. The PRACC service aims to pro-actively identify and address the palliative care needs of people in residential aged care facilities (RACFs) and support staff to develop and embed the skills and resources needed to provide quality palliative care in place. The Nurse Practitioner/Candidate position will be part of the Supportive and Palliative Care Service—one of the largest inpatient palliative care service in Victoria.
The successful applicant will be part of a small team but may work autonomously on a day to day basis. The role will involve engagement and collaboration with the Hospital-based Palliative Care Consult Service, the specialist inpatient palliative care unit located at Wantirna Hospital, The Eastern Health Inreach team, RACF staff, GPs and other community-based services. A palliative medicine specialist will also be part of the tea to provide clinical support. The Nurse Practitioner will be based between Wantirna Hospital and Wundeela (at Maroondah). Suitable clincical and professional supervision will be provided if a Nurse Pactitioner Candidate is appointed.
The objectives of the Nurse Practitioner/Candidate role are to work with a targeted group of RACFs to maximise residents’ quality of life, right up until death. This will involve:
The Nurse Practitioner will be accountable to the Associate Program Director and Clinical Director to achieve the PRACC Service’s KPIs. The PRACC trial will be conducted under the direction of a multidisciplinary steering group.
To be eligible for this position you will need to meet the following:
The successful applicant will have previous experience working as a nurse practitioner or in a senior clinical lead role and a demonstrated record of achievement of quality service delivery in palliative/end of life care and aged care. To be successful in the position you will need to be able to demonstrate the following attributes:
