Youth On Track | Youth And Family Case Manager

The Organisation

Our client is a community based, not for profit organisation that works to achieve social justice in regional Australia by supporting collaboration, encouraging action and helping people to take control and live well.

The Role 

The role will initially be a full time locum contract through until 30 June 2020, with potential for extension until 2022. 

Until 30 June, you will be paid at casual rates. If extended, you will become a direct employee of the organisation and be entitled to benefits including Salary Sacrifice. 

You will provide intensive, holistic case management, early intervention and outreach work to a case load of 10-12 participants. 

Youth on Track 

Youth on Track is an early intervention scheme for 10–17 year-olds that identifies and responds to young people at risk of long-term involvement in the criminal justice system.

Youth on Track provides the Police and Education with an opportunity to refer young people, known to be at medium to high risk of offending, to a support service without requiring a mandate. The young person's engagement in Youth on Track is voluntary.

Youth on Track has the benefit of multi-agency support. It aims to work collaboratively with other service providers to ensure consistent service provision without duplication of service. 

Key Responsibilities

  • Develop comprehensive case plans with the young person and their family
  • Deliver a range of evidence based interventions to address the risk factors
  • Develop effective working relationships with local community groups and stakeholders
  • Build relationships based on trust and respect and maintaining client confidentiality
  • Respond to referrals and conduct intake processes according to service guidelines
  • Record and submit accurate and detailed information to support program outcomes
  • Attend and actively participate in program training and courses related to Youth on Track

To Be Successful

  • Tertiary qualifications or equivalent experience in health, psychology, social work, social science or social welfare, health promotion or similar Social Science
  • Experience providing high quality customer-centred individual and/or family therapeutic interventions and/or case management
  • Excellent written and oral communication, interpersonal and negotiation skills, with the ability to communicate sensitively and effectively with all people to ensure effective relationships
  • Demonstrated well-developed organisational, time management and administrative skills with the ability to plan, prioritise and meet deadlines
  • Demonstrated experience in maintaining professional boundaries while engaging in person-centred work

Salary and Benefits

The role will initially be a full time locum contract through until 30 June 2020, with potential for extension until 2022. 

Until 30 June, you will be paid at casual rates - $42.56 per hour + Super.

If extended, you will become a direct employee of the organisation paid at SCHADS Level 4 and be entitled to benefits including Salary Sacrifice.
