The Clinical Nurse Consultant works within the Integrated Aged Care Team at Tamworth Community Health Centre. The role is to provide expert clinical advice and consultancy in the identification, assessment and treatment of delirium/dementia and support to staff working in acute care wards to build their capacity to effectively identify, assess and manage older people with cognitive impairment within a person centred care framework. Facilitate the development, implementation and evaluation of evidence based care management plans for patients with Delirium/Dementia, in liaison with other medical specialists, nursing and allied health staff. The Clinical Nurse Consultant will coordinate and assist the Acute Hospitals Educators to ensure all staff across the sector are educated on the management of delirium and dementia.
To be eligible for employment in these roles you must be: an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or a citizen of another country with an appropriate visa that allows you to work in Australia.
If you currently hold a temporary visa that allows you to live and work in Australia you may only be offered employment in line with the conditions and expiry date of your visa.
An eligibility list will be created for future permanent part time and temporary part time vacancies.
