Support Facilitator

An exciting opportunity is now available for an experienced Support Facilitator to join the welcoming and supportive team in Gympie.

The Position

This position will facilitate individualised care coordination and service navigation to clients of the Individual Recovery Support Program (IRSP). Delivering a recovery orientated & person centred approach service for clients with a Disability and / or Mental Health issue, the core responsibilities will include:

  • Manage the referral process, including assessing referred individuals
  • Work with clients to develop individualised recovery plans that identify clinical and non-clinical services and support partners
  • Facilitate initial engagement with services and support partners to assist with seamless transition
  • Source and engage with clinical and non-clinical services and support partners, developing sustainable partnerships and relationships
  • Work collaboratively with stakeholders to develop, monitor and review Recovery Plans
  • Contribute to the seamless implementation of transitional activities to develop the Individual Recovery Support Program
  • Promote an awareness of mental health / disability issues and STEPS Community Services throughout the wider community

The Successful Applicant

To be considered for this opportunity you must meet the following criteria:

  • Certificate IV in Mental Health or relevant qualification is essential
  • Demonstrated experience working in Mental Health Programs
  • Proven application of detailed knowledge, skills and ability in working with people with severe and persistent mental illness, using a recovery framework
  • Proven facilitation of individualised planning and support
  • The ability to develop strong relationships with fellow staff, clients and key stakeholders within the community
  • Hold open and clear communication abilities, both written and verbal
  • Have confident computer and reporting abilities

Applicants must also have the ability to complete a police check, hold a blue and yellow card and have a current driver’s license.
