MercyCare is seeking an experienced Case Worker for the Reconnect Program within our Youth and Homelessness department. This role is offered on a full time, contract basis with funding for the current program secured until 30 June 2021.
As a Case Worker for the Reconnect Program, you will provide an outreach service to young people aged 12-18 years and their family, which includes counselling, mediation, advocacy and practical support. The successful applicant will maintain a caseload of clients, participating in the referral intake process and ensuring person-centred, client-driven principles are integrated as part of case plans. You will provide both one on one support and group activities, maintaining accurate and comprehensive records and participating in events relevant to community needs.
